Application for Admission

The admission packet listed below includes all the forms you will need to apply for admission to Maplewood Academy. Specific instructions for these forms and other aspects of the admission process are outlined below. Students whose applications are received and whose files are completed early will be given first consideration where limitations exist. All applications will be reviewed and responded to as soon as they are received. Any additional questions you have can be answered by calling the Director of Recruiting at 320-587-2830.

Application Checklist Parent/Guardian

• Complete the Application for Admission (Form A). Be sure to provide all the information requested, printing legibly to ensure the accuracy of our information. Be sure to include all signatures, both parent/guardian and applicant, on all forms.

• Complete the Financial Agreement (Form C) in its entirety.

• Complete the Medical Consent and Health Record Forms (Form D & H).

• Collect all report cards, grades, and test results for your student since the beginning of his/her eighth grade year to present. These are required to ensure proper academic placement.
• Complete the Student Work Application (Form I)

• In the envelope provided, mail the following forms to Maplewood Academy: Application for Admission (Form A), Parent/Guardian Questionnaire (Form B), Financial Agreement (Form C), Medical Consent Form and Health Record (Form D & H), Student Questionnaire (Form E), Work Application (Form I), and the grades and testing information .

• Complete the Student Questionnaire (Form E) in your own handwriting.

• Give the School Report (Form F) to your current Principal, Teacher or Guidance Counselor. The individual completing the form must have known you for at least three months. Be sure to fill in your name on the form first and include one of the enclosed reply envelopes.

• You will need three Personal References (Form G) from adults who know you well (not a relative). Do not forget to provide them one of the enclosed reply envelopes.

When your application information is received and an enrollment decision is made, the Admissions Committee will officially notify you by mail. We appreciate your interest in Maplewood Academy and look forward to receiving your application.
